The Society currently wishes to appoint more 'Introducers' and thereby recruit new members. An 'Introducer' is likely to be an existing member, though need not be, and is a person who holds information and literature on our products and is able to introduce new members. Members can now download some of the forms used by the Society, and thus save time and money by not having to ring or write to us for these forms, and then to wait for the post. You can also download the terms of reference of our various committees for your information.

Increase of Rate Form - to use when applying for an increase in your monthly contributions to the Sickness Income Plan (2 pages).
Claims Form - to use when first notifying a Sickness claim (4 pages). Please note that sickness benefit is paid monthly in arrears.
Sickness Income Plan New Application form - for use by anyone you wish to introduce to the Society (don't forget to put your name on the application form so that you can get your £25 'member-get-member' reward), or by anyone wishing to join (4 pages). Please look at the Sickness Income Plan website page for important information regarding the documentation also required for anti-Money Laundering purposes.
ChildCare Application Form - for use by a parent or guardian to apply on behalf of a child aged 5 - 17 to join the ChildCare Plan.(3 pages). Please look at the 'ChildCare' website page for important information regarding the documentation also required for anti-Money Laundering purposes.
Tax-exempt Investment Policy Application Form - for use by anyone wishing to apply for a Tax-exempt Investment Policy.(2 pages)
Nomination Form - to be completed if you wish to nominate a person to receive up to £5,000 of the proceeds of your Fund(s) or policy immediately on production of the death certificate and without having to wait for Probate (2 pages).
Year 2007 Report and Annual Accounts is also available for downloading should you wish to have a copy (26 pages long, so it may take a while to download).
